641.2013 pg 1 , 641.2013 signature page 2 – All Small Business Development Centers throughout the country are required by the US Small Business Administration to collect this form from all clients who receive SBDC services. You can save time by completing and emailing/faxing it before attending a seminar or coming to your 1st one-on-one counseling appointment.
Business Proposal Assessment – The business proposal assessment helps your business advisor understand your business idea and the nature of the assistance you are seeking from the SBDC. You can save time by completing it before your first one-on-one counseling session.
